Job summary

NTSOC is seeking a Field Nurse Case Manager to oversee admissions, recertifications, and coordination of home health nursing for clients in southern Colorado. You will supervise field staff, provide education, and ensure compliance with state and federal regulations.

The role blends field visits with remote coordination to support families and multidisciplinary teams. The position reports to the Assistant Director of Nursing and offers a competitive salary and comprehensive benefits.

Qualifications

• Graduate of an Accredited Nursing Program; BSN preferred.
• 1 year home health care experience preferred and 1-2 years in a health care setting required.
• Strong communication and computer skills.

Responsibilities

• Provides comprehensive assessment of patients to determine eligibility and continued eligibility.
• Reports significant findings to the appropriate physician.
• Submits documentation in a timely, complete and accurate manner.
• Implements physician’s orders in a timely, complete and accurate manner.
• Plans, coordinates and implements the patient’s plan of care in compliance with Agency and governmental regulations.
• Performs patient care visits in accordance with Agency productivity requirements.
• Creates and maintains accurate field charts to facilitate continuity care.
