Job summary

Humana is seeking a Field Care Manager II (RN) to assess member needs and guide families toward appropriate resources. You will work with the Louisiana Medicaid team, develop care plans, and coordinate with providers to support physical health and psychosocial wellbeing.

You will conduct assessments, monitor progress, and document outcomes while operating within a remote/home office setting and traveling up to 50% within Louisiana.

Qualifications

• Active, unrestricted RN license in Louisiana.
• 2+ years of clinical experience with diverse populations.
• Knowledge of community health and social service agencies.
• Experience with EMRs and narrative documentation.
• Proficient in Microsoft Word, Outlook and Excel.
• Ability to work independently with sound judgment and adapt care plans.

Responsibilities

• Engage and empower members and families using holistic strategies.
• Conduct telephonic and in-person assessments to evaluate needs and guide to resources.
• Monitor member progress and adjust care plans accordingly.
• Develop personalized care plans based on clinical judgment and psychosocial factors.
• Collaborate with healthcare providers and community services to ensure quality care.
• Coordinate delivery of essential services addressing physical health and social determinants.
• Facilitate communication across the care team for seamless transitions of care.
• Align daily practices with department and organizational goals.
• Document and submit incident reports per policy.
