Job summary

Humana is seeking a compassionate RN, Field Care Manager 2 (Physical Health) to assess member needs, develop care plans, and coordinate services across providers. Qualifications

• Active, unrestricted RN license in Louisiana.
• 2+ years of clinical experience with diverse populations.
• Knowledge of community health and local resources.
• Experience with EMR, narrative documentation, and web navigation.
• Proficient in Word, Outlook, and Excel.
• Ability to work independently and adapt care plans.

Responsibilities

• Engage and empower members and families using holistic strategies.
• Conduct telephonic and in-person assessments to guide to resources.
• Monitor member progress toward individualized care goals.
• Develop and implement personalized care plans based on clinical judgment.
• Collaborate with providers and community services for cost-effective care.
• Coordinate delivery of essential services addressing health needs and social determinants.
• Facilitate communication across the interdisciplinary care team for seamless transitions.
• Align daily practices with departmental and organizational objectives.
• Document and submit incident reports per policies.
