Job summary

Humana is seeking a RN Field Care Manager Nurse 2 (Maternal-Child Health) to assess member needs and guide families to appropriate resources. This remote field role emphasizes independent judgment, travel within Northern Louisiana, and collaboration with PCPs and community services to ensure high-quality outcomes.

You will develop care plans, monitor progress, and participate in health promotion activities while maintaining compliance with LDH directives and Humana policies.

Qualifications

• Active RN license in Louisiana without restrictions.
• Two or more years of clinical experience in maternal-child health, Labor & Delivery and/or pediatrics.
• Proficient with electronic medical records and MS Word/Outlook/Excel; strong communication skills.

Responsibilities

• Identify and resolve barriers to effective care.
• Engage members and families using a holistic, member-focused approach.
• Conduct telephonic and in-person assessments to identify needs and guide resources.
• Monitor progress and support optimal health and wellbeing.
• Develop individualized care plans using clinical judgment.
• Collaborate with providers and community services for quality outcomes.
• Coordinate services across physical health, social determinants of health, and benefits.
• Facilitate communication across care team including PCP and transitions.
• Provide resources to families for health and wellness support.
• Submit incident reports as required.
• Participate in health promotion and community events.
