Ideal candidates have 2+ years in healthcare with care management exposure and an active RN license. Travel 25–40% and proficiency with Microsoft Office are required; the role operates Mon–Fri, 8 AM–5 PM CST.

Qualifications

• At least 2 years of healthcare experience with care management exposure.
• RN license must be active and unrestricted in state of practice.
• Valid driver’s license and reliable transportation for travel requirements.
• Ability to work independently across varied settings and with diverse populations.
• Proficiency in Microsoft Office applications and documentation.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including home visits as required.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and the care team.
• Monitor care plans, document interventions, and adjust as needed.
• Promote integration of LTSS and behavioral health services; coordinate resources.
• Assess medical necessity and authorize waiver services; conduct medication reconciliation as needed.
• Travel 25-40% locally as required by state/contractual needs.
