Qualifications

• Proficient in EMR and MS Office.
• Ability to work with minimal supervision.
• Hybrid home/market office with travel across Illinois.

Responsibilities

• Identify and resolve barriers to care.
• Monitor patient progress through assessments.

• Understand organizational objectives and linkages to care.
• Make pragmatic decisions with minimal guidance.
• Follow established guidelines and procedures.
