The position requires a Bachelor’s degree in a health field (Social Work, Nursing, or similar) with 2+ years of caseload management, strong organization, and travel up to 75%.

Qualifications

• Bachelor’s degree in Social Work, Nursing or other health care profession including LPN.
• 2+ years of clinical case management experience.
• 1+ years of experience with MS Office, including Word, Excel, and Outlook.
• Reside in or within a commutable driving distance to Washington county.
• Access to reliable transportation and valid US driver’s license with travel up to 75% within an assigned territory to meet with members and providers.

Responsibilities

• Coordinate member care needs across Medicare/Medicaid benefits.
• Deliver member-centered, self-management support and education.
• Ensure appropriate utilization and consistent application of health benefits.
• Conduct telephonic and face-to-face assessments per policy and timeframe.
• Collaborate with Care Coordination team to improve care quality and efficiency.
