RNs are responsible for conducting (hands-off/in-field (reimbursement rate for mileage = .535/mile)) assessments for a state mandated tool. Assessments will take approximately 2.5-3 hours per assessment. Will complete approximately 2 cases per day (10/week).
Assessments evaluate ADL (activities of daily living) needs and determine if the member meets requirements for nursing facility care. Assessments will be conducted in the member's homes, nursing facilities, and medical day care facilities.
Will travel primarily in certain counties but should be open and willing to travel throughout NJ, as needed. Assessments are conducted in the field but follow-up work can be done remotely from home.
RNs will be given an iPad to complete assessments.

Responsible for overall management of the member's case within the scope of licensure; provides supervision and direction to non-RN clinicians participating in the member's case in accordance with applicable state law and contract; develops, monitors, evaluates, and revises the member's care plan to meet the member's needs, with the goal of optimizing member health care across the care continuum.
