Memorial Medical Center in Las Cruces, NM, part of Lifepoint Health, is seeking an RN to join the discharge planning team. You will coordinate with payers and community services to arrange post-hospital care, housing, transportation, and related services.

Your role focuses on patient transitions and ensuring continuity of care. The position requires an RN license in the state and an associate's degree; CCM/ACM certifications are desirable and relocation/other benefits apply.

Qualifications

• Current RN license in the state
• Associate's degree from accredited nursing program
• Licensure required for professional affiliation
• CCM/ACM certifications desirable

Responsabilités

• Assist with discharge planning and ensure smooth patient transitions
• Coordinate with third-party payers, government programs and community services
• Arrange appropriate care, housing, transportation and DME
• Support social services and end-of-life planning
• Navigate complex discharge processes to maintain continuity of care
