Job summary

Qualifications

• Current RN licensure required in the state.
• Certification by National Association of Healthcare Professionals or InterQual is desirable.
• Certification as CCM or ACM desired.

Responsibilities

• Coordinate discharge planning with third-party payers, government programs, and community services.
• Arrange appropriate care, housing, transportation and other essential services for patients.
• Assist with long-term care, rehabilitation, behavioral/mental health, home care, and DME referrals.
• Support social services in discharge planning and end-of-life planning.
