Job summary

Senior Community Care of Michigan PACE in Lansing, MI is seeking an Enrollment Specialist to join our care‑driven team. You will manage referrals, assess potential participants, and guide families through intake and enrollment in our PACE program.

Bachelor’s degree in humanities/health and human services or equivalent with CNA or LPN certification; two years in healthcare marketing or admissions; valid driver’s license and transportation within 100 miles. Monday–Friday 8:00 AM–4:30 PM.

Qualifications

  • Bachelor’s degree in humanities/health and human services or equivalent with CNA or LPN certification.
  • Two years of experience in marketing, with emphasis on sales/admissions in a health care setting.
  • Shall have either training or related experience in the job assigned.
  • Must have a valid driver’s license and transportation within 100 miles daily.

Responsibilities

  • Receives and provides information to referrals to the Senior Community Care Program; refers individuals to other appropriate resources.
  • Assesses potential participants in home, nursing homes, hospitals or other settings as part of the intake process.
  • Maintains current, written assessment and intake records inclusive of potential participant’s wishes and the services provided.
  • Assists potential participants in managing their needs during intake and assessment, including referrals to doctors and community agencies.

Pay Range: $54,400-$60,900

Schedule: Monday-Friday 8:00 AM-4:30 PM

What You Bring (Requirements)
  • Bachelor’s Degree in humanities/health and human services or equivalent experience with C.N.A. or LPN certification.
  • Two (2) years of experience in marketing, with more emphasis on sales, and/or admissions role for a health care provider
  • Shall have either training or related experience in the job assigned.
  • Must have a valid driver’s license and have means of transportation within one hundred (100) miles or more daily.
What You’ll Be Responsible For
  • Receives and provides information to referrals to the Senior Community Care Program; refers individuals to other appropriate resources as indicated.
  • Assesses potential participants in home, nursing homes, hospitals or other settings as part of the intake process.
  • Maintains current, written assessment and intake records inclusive of potential participant’s expressed wishes and the referral services provided, as appropriate.
  • Assists potential participants in managing their needs while undergoing intake and assessment process, including referrals to doctors and other community agencies.

As a Program of All-Inclusive Care for the Elderly (PACE) provider, our interdisciplinary teams deliver personalized, comprehensive care tailored to each participant’s needs.
