Job summary

Saint Mary’s Health Network, a member of Prime Healthcare, is seeking an experienced RN to coordinate discharge planning within the Case Management Department in Reno, NV. This full-time role supports safe patient transitions and collaborates with an interdisciplinary team to optimize care and resource use.

The ideal candidate holds Nevada RN licensure and BLS certification, with hospital-based discharge planning knowledge and strong communication, critical thinking, and meticulous follow-up

Qualifications

• Knowledge of Discharge Planning/Utilization Management/Case Management terminology and functions.
• Experience with basic to intermediate computer skills.
• Excellent verbal and written communication and ability to collaborate with an interdisciplinary team.
• Ability to manage multiple complex cases with attention to detail and follow-up.
• Current RN licensure in Nevada and BLS certification.
• Hospital experience preferred.

Responsibilities

• Coordinate activities of the Case Management Department under the direction of the Case Manager/Social Worker.
• Assist with development and implementation of discharge plans, case management referrals and authorizations.
• Maintain and coordinate Medicaid state submission forms for on-site review.
• Assist with discharge planning by interviewing patients and families to develop a safe discharge plan.
