Job summary

Saint Mary’s Health Network in Reno, NV is seeking a Nurse Case Manager to coordinate case management activities and assist with discharge planning. The role requires RN licensure in Nevada and BLS certification, with prior hospital experience preferred.

You will work with a multidisciplinary team to develop safe discharge plans and ensure smooth transitions of care, while navigating Medicaid submissions and referrals under the supervision of an assigned Case Manager.

Qualifications

• Knowledge of Discharge Planning/Utilization Management/Case Management terminology and functions, in both managed care and non-managed care environments.
• Experience with basic to intermediate computer skills.
• Demonstrates excellent communication skills both verbal and written, ability to facilitate and collaborate with an interdisciplinary team, flexibility to manage multiple complex cases, attention to detail and follow-up, critical thinking and problem-solving skills.

Responsibilities

• Coordinate the activities of the Case Management Department under the direction of the Case Manager/Social Worker.
• Assist with development and implementation of discharge plans, case management referrals and authorizations.
• Maintain Medicaid state submission forms for presentation to on-site reviewer and file them accordingly.
• Assist with Discharge Planning by interviewing patients/families and gathering data to develop a safe patient-focused Discharge Plan.

Required Qualifications

• Current licensure as a Registered Nurse in the State of Nevada
• Current BLS (AHA) certificate
• Experience in the hospital setting preferred.
