Job summary

Qualifications

• Must have working-level English including reading, writing and speaking.
• Understanding of medical terminology preferred.

Responsibilities

• Coordinate discharge plans for patients with RN Case Manager and Social Workers.
• Attend unit rounds daily to receive direction and report progress on discharge planning.
• Initiate referrals and follow-up calls for home care, infusion, wound care, DME, SNF and hospice as requested.
• Arrange transportation for patients as needed.
• Document all discharge planning activities in the medical record.
