Job summary

CHI St. Alexius Health Turtle Lake is seeking an RN Care Coordinator to oversee care progression and discharge planning, ensuring quality outcomes and adherence to policies.

You will coordinate with utilization management, nursing, physicians, insurers, and post-acute providers to advance care to the appropriate level and advocate for patients and families. The role requires strong clinical judgment, excellent communication, and a commitment to patient advocacy and experience, with an incentive

Qualifications

• Graduate of an accredited school of nursing.
• RN license in North Dakota or Compact state.
• BLS certification within 3 months of hire.
• Two years acute hospital clinical experience or CCM/UM credentials (Masters in lieu).
• BSN preferred for advancement.

Responsibilities

• Discharge assessment and planning documented to meet clinical and psychosocial goals.
• Lead discharge plans with the care team, patients, families, and post-acute providers.
• Coordinate progression of care using evidence-based criteria.
• Facilitate transitions to post-acute services and reduce readmission risk for 30 days.
• Educate patients and advocate for their care; ensure notifications and accurate documentation.
• Maintain compliance with policies and participate in performance improvement.
