Responsibilities

• Collaborate with interdisciplinary teams to coordinate patient care.
• Develop and implement discharge plans and post-hospital care arrangements.
• Refer cases when criteria are not met and ensure timely interventions.
• Serve as liaison with community agencies to ensure smooth transitions.
