Qualifications

• Current clinical licensure strongly preferred.
• One year recent relevant experience required. Acute care setting preferred.

Responsibilities

• Develops a plan of care with physicians and the multidisciplinary team for assigned patients.
• Monitors patient progress through assessments and records to ensure desired outcomes.
• Identifies and resolves barriers hindering effective patient care.
• Actively participates in discharge planning.
• Aligns daily practice with the hospital’s mission and guiding principles.
