Job summary

The New York Foundling is seeking a Registered Nurse to deliver high-quality, person-centered medical care within the Developmental Disabilities Division. You will assess health needs, coordinate care, and train staff to ensure safety and regulatory compliance.

Responsibilities include conducting health assessments, administering medications, triage, and reducing hospitalizations, while maintaining thorough documentation and collaborating with physicians and families.

Qualifications

  • Current New York State RN license in good standing.
  • 1+ years of nursing experience.
  • Experience with individuals with developmental disabilities in residential or community settings (strongly preferred).
  • Strong clinical assessment skills to identify, monitor, and respond to medical conditions and changes in health status.
  • Ability to educate and train staff on health-related topics, infection control, emergency protocols, and individualized medical needs.
  • Strong organizational and documentation skills, with the ability to maintain accurate health records and support compliance with OPWDD standards.
  • BLS certificate.
  • Demonstrated strong commitment to safety.

Responsibilities

  • Assess persons served medical conditions to develop targeted care plans that prevent illness, manage chronic conditions, promote long-term well-being and advocate for the rights and well-being of individuals served.
  • Conduct regular health assessments and nursing reviews. Administer medications and treatments as prescribed; monitor for adverse reactions and report appropriately. Track and analyze health trends, hospitalizations, and incidents to address health concerns.
  • Coordinate with physicians, DPSs, families and other healthcare providers to align care efforts and reduce gaps in medical management.
  • Provide prompt and accurate telephone triage and consultation to persons served and staff, ensuring urgent health concerns are quickly identified and effectively managed.
  • Work with the care team to reduce avoidable hospitalizations or ER visits.
  • Ensure accurate, timely, and thorough documentation of all nursing activities and maintain compliance with state and federal regulations.
  • Train and oversee unlicensed staff (e.g., DSPs) in medication administration, health monitoring, infection control, and emergency protocols; provide AMAP certification and recertification training.
  • Maintain Medication Safety: Oversee medication administration processes to ensure compliance with regulations and minimize errors.
