Qualifications

• Active NYS RN license.
• Bachelor’s Degree in Nursing preferred.
• PMCS, BLS certifications.
• Minimum 2 years' experience in a medical/surgical setting.

Responsibilities

• Coordinate a patient-focused team in providing direct and collaborative health care across clinical settings.
• Utilize the nursing process to reflect understanding of diverse patient rights and needs.
• Apply case management knowledge to provide comprehensive, coordinated care across the continuum.
• Coordinate, integrate, and facilitate the patient/provider relationship throughout care.
