Job summary

Solero Behavioral Transitions in Pickerington, OH is seeking a dedicated case manager to support adults navigating substance use and mental health treatment. The role follows a Monday–Friday schedule (8:00 am–4:30 pm) in a structured, outpatient setting.

You will perform assessments, develop service plans, coordinate care with clinicians, and help clients access housing, employment, and healthcare resources. Strong communication and advocacy are essential to success.

Qualifications

• Active CDCA certification in Ohio is required.
• High School Diploma or GED required.
• Must be 21 years of age or older.
• Newly qualified grads welcome.

Responsibilities

• Provide case management including assessment, service planning, and ongoing support.
• Use peer support to engage clients in recovery.
• Assist clients with housing, employment, and healthcare access.
• Maintain timely documentation in line with state guidelines.
• Coordinate care with clinical staff and support treatment goals.

Solero Behavioral Transitions in offers a safe, structured environment for adults managing substance use and mental health challenges. As a dual diagnosis outpatient program, we provide integrated treatment for individuals with primary substance use or mental health disorders.

Key Responsibilities

• Provide case management services, including assessment, service planning, and ongoing support
• Utilize peer support principles to engage and motivate clients in recovery
• Assist clients in accessing community resources such as housing, employment, and healthcare
• Maintain accurate and timely documentation in compliance with agency and state guidelines
• Collaborate with clinical staff to coordinate care and support treatment goals
• Facilitate individual and group sessions as appropriate
• Advocate for clients and promote a recovery-oriented environment
