Job summary

Benchmark Human Services in Acworth, GA seeks a Registered Nurse (RN) to provide healthcare coordination and direct intervention for residents in our Crisis Support Home program. The RN will collaborate with mobile crisis teams and provide clinical oversight for unlicensed staff to meet healthcare needs.

The role requires an active Georgia RN license, experience with IDD/mental health nursing, and ability to train staff on medication supervision, health policies, and monitoring.

Qualifications

• RN license in Georgia is required.
• Experience with IDD and mental health nursing preferred.
• Experience with individuals who have intellectual/developmental disabilities and/or mental illness.

Responsibilities

• Provide healthcare coordination and direct intervention for individuals in accordance with state and federal regulations.
• Liaise and advocate for individuals’ healthcare, wellbeing, and inclusion.
• Train unlicensed staff on medication supervision, delegated tasks, and health protocols.
• Oversee pharmacy and medication needs and maintain health-related records.
• Assist with transitions of care to/from hospital, nursing home, or rehab facility.
• Respond to emergency medical situations promptly and attend medical appointments as directed.
