Job summary

Benchmark Human Services in Acworth, GA is seeking a Registered Nurse (RN) to provide healthcare coordination and direct intervention for individuals in our Crisis Support Home program. The RN will collaborate with mobile crisis teams, train unlicensed staff, and provide clinical oversight of LPNs.

This role serves young adults with autism and IDD, ensuring compliance with state and federal regulations, managing transitions of care, and responding to medical emergencies.

Qualifications

• RN license active in Georgia.
• Experience in IDD/Mental Health nursing preferred.
• Experience working with individuals with intellectual and developmental disabilities and/or mental illness.

Benchmark is seeking a Registered Nurse (RN) to provide healthcare coordination and direct intervention for individuals in our Crisis Support Home program in Acworth GA. The group home serves young adults with autism and other forms of IDD. The RN will collaborate with Benchmark's mobile crisis teams. The Nurse will provide training and clinical oversight for unlicensed staff to meet the healthcare needs of individuals served. As well as provide clinical oversight of program LPNs, as applicable.

Schedule: Mon-Fri 9am-5pm + on call

Responsibilities:

• Provide healthcare and wellness coordination for individuals in accordance with all state and federal regulations.
• Function as liaison and advocate for individuals' healthcare, wellbeing, and inclusion.
• Train unlicensed staff on medication supervision of self-administration, delegated tasks, treatment protocols, and monitoring.
• Implement and train employees on new and revised health related policies and procedures
  in a timely manner as directed.
• Oversee pharmacy and medication needs.
• Maintain health-related records, complete documentation and assessments.
• Obtain and maintain HRST rater certification.
• Support individuals in obtaining health services appropriate for their needs.
• Oversee that the health status and physical conditions of individuals are assessed, monitored, reported, documented, and responded to in a timely and appropriate manner.
• Respond to emergency medical situations in a timely manner, as appropriate.
• Participate in and attend assigned medical individuals' appointments (as directed).
• Develop and update individual specific plans of care and health care plans to ensure appropriate medical care and supervision for assigned individuals.
• Manage transitions of care to and from hospital, nursing home, rehab facility and/or another agency.
• Mandatory reporter of any suspected abuse, neglect, or exploitation to the supervisor or

Qualifications:

• RN with current Georgia license.
• Experience in IDD/Mental Health nursing is preferred.
• Experience working with individuals with intellectual, developmental disabilities and/or mental illness.
• Valid Driver's License, with no driving limitations.

Salary Description
