Job summary

Benchmark Human Services in Augusta, GA seeks an RN Healthcare Coordinator to provide healthcare coordination and direct intervention for adults with Intellectual and Developmental Disabilities in transitional homes.

The RN will train unlicensed staff, oversee LPNs as applicable, maintain health records, and respond to emergencies while supporting individuals in obtaining appropriate health services, all within a mission-driven, diverse workplace.

Qualifications

• RN license required and current in Georgia.
• Prior IDD/Mental Health nursing experience preferred.
• Experience with individuals with intellectual/developmental disabilities and/or mental illness.
• Valid driver’s license with no driving limitations.

Responsibilities

• Provide healthcare and wellness coordination for individuals in accordance with state and federal regulations.
• Liaise and advocate for individuals’ healthcare, wellbeing, and inclusion.
• Train unlicensed staff on medication supervision, delegated tasks, protocols, and monitoring.
• Implement and train on health-related policies and procedures as directed.
• Oversee pharmacy and medication needs; maintain health records and assessments.
• Obtain HRST rater certification; ensure timely health services for individuals.
• Monitor health status and respond to emergencies promptly.
• Attend medical appointments as directed; assist in care planning.
• Manage transitions of care to/from hospital, nursing home or rehab facilities.
