Qualifications

• BSN required; MSN preferred.
• 3 years in healthcare navigation, concierge, or guest relations.
• Active RN license in NJ.
• Strong customer service and HIPAA knowledge.

Responsibilities

• Initiate proactive communication with patients to assess needs.
• Develop and implement individualized care plans based on assessments.
• Serve as patient liaison and primary contact for outpatients.
• Coordinate transitions between ambulatory and inpatient settings.
• Conduct follow-up calls for post-acute care and recovery.
• Assist with discharge planning and care coordination.
• Maintain HIPAA‑compliant documentation and communications.
