Job summary

Capital Health in New Jersey seeks a registered nurse for home health care to assess participants, develop plans of care and coordinate with the interdisciplinary team to ensure high-quality home-based care.

The role requires an ASN or RN license, BLS, and NJ RN licensure; BSN is preferred. Prior home care experience is desirable, with a strong focus on education and patient advocacy.

Qualifications

Responsibilities

• Triages and assesses participants in the home clinic setting and provides education resources.
• Participates in the interdisciplinary team (IDT) and develops care plans.

• Educates participants and families regarding services and care needs.
• Collaborates with Clinic RNs, RN Case Managers, and PCPs for transitions of care.
• Participates in Quality Committee activities and supports QAPI initiatives.
• Conducts comprehensive nursing assessments during status changes and as needed.
