Job summary

Join the Cleveland Clinic West Palm Beach team as an RN in Concierge Medicine, providing patient-first care during weekend days on a 8:00 am – 8:30 pm shift. You will collaborate with a multidisciplinary team to coordinate care across the patient journey.

You will educate patients and families, support transitions of care, and apply disease management best practices to improve outcomes in a high-quality, respected healthcare system.

Qualifications

• Graduate from an accredited school of Professional Nursing.
• Current state licensure as a Registered Nurse (RN).
• Basic Life Support (BLS) certification through AHA or Red Cross.
• Three to five years of nursing experience.

Responsibilities

• Collaborate with a multidisciplinary care team to develop goals and plans for high-risk patients.
• Provide care coordination and disease management.
• Educate patients and families about care plans and self-management.
• Support transitions of care across care settings and coordinate with care teams.
