Job summary

The Cleveland Clinic West Palm Beach is seeking a dedicated RN for the Rapid Response Program. You will deliver professional nursing care across ambulatory settings while supporting caregivers in providing exceptional patient experiences.

You will collaborate with ER physicians to respond to acutely ill patients and coordinate transfers to Weston Hospital, serving as a key liaison for timely care. You will participate in prior authorizations, telephone triage, and other duties as needed, with

Qualifications

• Graduate from an accredited School of Nursing (ADN or BSN).
• Current state licensure as a Registered Nurse (RN).
• Basic Life Support (BLS) through AHA or Red Cross.
• ACLS within six months of hire where applicable.
• One year of recent RN experience or equivalent ambulatory care experience within Cleveland Clinic.

Responsibilities

• Assess health status by completing a nursing assessment.
• Perform telephone triage.
• Notify physicians of abnormal findings.
• Establish a care plan with patient and health team.
• Collaborate with care coordinators for smooth transitions of care.
• Administer medications and monitor responses.
• Initiate CPR and emergency measures.
• Provide health education to patients and families.
• Document education to learning needs.
• Participate in quality initiatives and research to improve outcomes.
