Job summary

The ideal candidate has a BSN and at least 3 years of acute care experience, including 1+ year in discharge planning or case management, plus proficiency in Epic EHR.

Qualifications

• BSN required.
• 3 years of clinical experience in acute care with 1+ year in case management or discharge planning.
• Experience with leased bed programs, patient throughput management, or transitional care coordination preferred.
• Experience with diverse and underserved populations preferred.

Responsibilities

• Manage patient flow from hospital to post-acute settings to reduce length of stay.
• Coordinate discharge planning and bed leasing as needed to ensure timely transitions.
• Support access to benefits and resources for safe discharge and long-term success.
• Maintain strong relationships with physicians, interdisciplinary teams, patients, families, and community partners.
