Job summary

UCSF Health is seeking an experienced Case Manager to promote optimal clinical outcomes and manage complex inpatient caseloads. You will coordinate discharge planning and contribute to program development within an interdisciplinary team.

The role requires strong communication, analytical skills, and proficiency with electronic health records, along with the ability to work across UCSF Medical Center locations.

Qualifications

• Minimum of two years inpatient nursing case management experience.
• Bachelor’s Degree in a healthcare related field.
• California RN license.
• Strong written and oral communication, diplomacy, organizational and analytic skills.
• Ability to work effectively with interdisciplinary teams.

Responsibilities

• Promotes optimal clinical and resource outcomes and facilitates appropriate lengths of stay.
• Oversees coordination of complex discharge planning for a designated caseload.
• Acts as a consultant to clinical teams and participates in program development.
• Provides age-appropriate assessments, data interpretation, and delivery of interventions.
