Job summary

University of Michigan's Michigan Medicine in Ann Arbor is seeking a Registered Nurse for the Primary Care - RN Complex Care Navigator role. You will identify high-risk patients, assess needs, collaborate with the care team, and develop individualized care plans to improve outcomes.

Qualifications

• Current Michigan license as a Registered Nurse.
• Two years of experience with complex adult and pediatric patients.
• Two years in case management, IP or ambulatory care.
• Knowledge of chronic conditions management and health risk assessment.

Responsibilities

• Identifies the targeted high-risk population within practice site(s), risk stratification, and patient lists.
• Assesses health care, educational, and psychosocial needs over time using standardized tools.
• Collaborates with providers and interdisciplinary team to develop and monitor an individualized plan of care.
• Provides patient self-management support to empower patients and families.
• Coordinates care across settings and ensures continuity of care beyond the practice.
