Qualifications

• RN license required.

Responsibilities

• Facilitate timely discharges and safe care transitions.
• Collaborate with interdisciplinary teams and mentor new staff/precept interns.
• Identify barriers to timely disposition and document assessments/interventions.

• Conduct family meetings and integrate cultural or religious considerations.
