Qualifications

• Associates or Diploma in Nursing; BSN preferred.
• Current RN license; BLS certification required.
• Minimum 3 years healthcare experience; 2 years in care coordination.

Responsibilities

• Assess complex patients' psychosocial risk factors and discharge needs.
• Coordinate care across the continuum with the care team.
• Lead interdisciplinary rounds and support family decision-making.
• Mentor new hires and provide precepting as needed.
• Document care plans and maintain accurate EHR records.
