Job summary

Wellstar Cobb Hospital is seeking a RN Complex Care Coordinator to assess complex patient needs, coordinate care across the continuum, and ensure timely, safe discharge planning with family engagement. You will serve as a consultant to care teams on clinical, psychosocial, and resource needs.
