Job summary

Wellstar Cobb Hospital is seeking an RN Complex Care Coordinator to assess complex patient needs, coordinate care across the continuum, and assure safe discharge planning with the care team. The role partners with physicians and the care team to tailor transitions and resource support for high-risk patients.

Responsibilities include psychosocial assessments, coordinating community resources, and mentoring new coordinators.

Qualifications

• Experience with complex patient populations and discharge planning.
• Ability to coordinate across care teams.
• Experience in performing psychosocial assessments.

Responsibilities

• Implements discharge planning and provides resource information for complex patients.
• Identifies barriers for timely disposition and documents them.
• Participates in Interdisciplinary Rounds to confirm discharge date and care level.
• Initiates post-acute referrals to ensure timely transition to next level of care.
• Provides financial needs assessment and supports patient/family decision making.
• Precepts and mentors new care coordinators as needed.
