BMCHS in the Boston area is seeking a Complex Care Manager RN to support primary care-based, multi-disciplinary care management for high-need patients. The role is full-time (40 hours/week) at a clinical site, with a salary/incentive plan and a focus on chronic disease management and social determinants of health.

Responsibilities include patient recruitment, care plan development with the care team, and ongoing coordination with community partners, ensuring HIPAA compliance and program metrics.

Formación

• Nursing degree required with ability to obtain BSN within 4 years.
• Licensed RN in Massachusetts or eligible to practice.
• CCM Certification preferred.
• Two years clinical care management experience preferred.

Responsabilidades

• Identify and recruit appropriate patients for care management.
• Develop patient-centered care plans focused on chronic disease management and social determinants.
• Collaborate with care team and community partners.
• Document and adhere to HIPAA and performance metrics.
• Meet regularly with site leaders and supervisors.
• Work full-time at the clinical site.
