Job summary

West PACE in California seeks a Clinic RN to coordinate direct and indirect nursing care for participants at the center and in their homes. You’ll work closely with the PACE Physician, clinic staff, and the interdisciplinary team to manage care plans and support the clinic’s mission.

The role requires an accredited nursing degree, CA RN license, BLS/First Aid, and a valid driver’s license. Two years of nursing experience with frail/elderly populations is preferred, with bilingual Spanish-English

Qualifications

• Graduate of an accredited school of nursing with CA RN license.
• BSN preferred; current BLS/First Aid certification.
• California RN license required; driver’s license and auto insurance proof.

Responsibilities

• Conduct Clinic Nursing assessments and participate in IDT meetings.
• Visit participants' homes to deliver nursing interventions.
• Administer medications and perform ordered tests and treatments.
• Maintain accurate documentation and protect privacy of participants and families.
• Coordinate with PACE Physician and IDT for smooth care transitions.
• Instruct participants, family and caregivers on disease processes and prevention.
