Job summary
PacificSource PACE LLC seeks a Registered Nurse to deliver direct clinic nursing care within a geriatric PACE clinic. You will assess patients, administer vaccines, support care plans, and coordinate with physicians, nurse practitioners, social workers, and other team members to ensure compassionate participant care.
Qualifications include 3+ years nursing with frail older adults, current RN license, Associate degree, and CPR certification.
Qualifications
- 3+ years nursing experience with frail, older adults.
- Active RN license in state.
- CPR certification at hire or within 3 months.
Responsibilities
- Provide direct clinic nursing care (vital signs, injections, wound care).
- Educate participants and caregivers on treatments and preventive health.
- Coordinate follow-up and support diagnostic and treatment plans.
- Act as clinical resource and support interdisciplinary team.
