Job summary

The Center for Human Development (CHD) in Holyoke, MA is seeking a Registered Nurse (RN) to support Adult Community Clinical Services (ACCS). The RN coordinates health-related activities, conducts assessments, participates in medical reviews, and provides counseling on health and behavioral well-being in community settings.

Ideal candidates have a Bachelor's degree, 2+ years in mental health or substance use, MAP experience and a valid driving record.

Qualifications

• Bachelor's degree preferred
• Two years of experience working within mental health and/or substance use field is preferred
• MAP experience preferred
• Residential experience preferred
• Acceptable driving record preferred

Responsibilities

• Coordinate health-related activities to maintain and promote the health of Persons served.
• Assess needs, participate in medical reviews, and provide counseling in health and psycho-physiological well-being.
• Liaise with other medical and community-based providers to coordinate care.
• This is an administrative RN role within ACCS.
