Job summary

Option Care Health in the Philadelphia area seeks a Clinical Transition Educator (RN) to provide clinical education and training, ensuring smooth patient transitions to home or alternate sites. The role partners with the sales team to expand patient transitions to our care delivery model.

You will conduct patient assessments, educate patients and caregivers, coordinate with facilities and internal teams, and document education using our applications.

Qualifications

• Active and unrestricted RN license required.
• Minimum 2 years of experience in the healthcare industry.
• Experience building relationships with diverse stakeholders.
• Proficient PC skills: Word, Excel, PowerPoint, email/internet.

Responsibilities

• Educate patients, caregivers and facility staff about home or alternate-site infusion services.
• Conduct patient assessments to determine suitability for the OCH care model.
• Collaborate with sales to identify educational needs and improve transition processes.
• Document education using OCH applications and technology with confidentiality.
• Coordinate hospital-to-home transitions with facilities, providers and internal teams.
