Job summary

Option Care Health is seeking a Clinical Transition Educator to provide clinical education and training for patient transitions to our care delivery model. You will partner with the sales team to boost patient transitions and ensure smooth in-home or alternate-site care.

The role requires an RN license, at least 2 years in healthcare, and ability to travel locally to care facilities, hospitals, and community settings. Strong documentation and communication skills are essential.

Qualifications

• Active and unrestricted RN license required.
• Minimum of 2 years in healthcare.
• Experience establishing relationships with stakeholders.

Responsibilities

• Educate patients, caregivers, and facilities on OCH services in home/alternative sites.
• Conduct patient assessments to determine suitability for the care model.
• Collaborate with sales to identify educational needs and improve transition volume.
• Maintain confidentiality of patient information and comply with legal guidelines.
• Document education using OCH applications and technology.
• Coordinate hospital-to-home transitions with facilities and internal teams.
