Qualifications

Responsibilities

• Educate patients, caregivers, and facility staff on OCH services and home infusion delivery model.
• Assess patient suitability for home-based care and coordination with clinical teams.
• Collaborate with sales to identify education gaps and drive patient transitions.
• Document education activities in OCH systems and ensure confidentiality.
