The ideal candidate holds an active RN license, has at least two years in healthcare, and can educate patients, caregivers, and facility staff across in-home, hospital, and virtual settings.

Qualifications

• Active and unrestricted RN license required.
• Minimum of 2 years of experience in the healthcare industry.

Responsibilities

• Evaluate, educate, and train patients, caregivers, and facility staff about OCH services and products for seamless transition to home care.
• Conduct patient assessments to determine viability for the OCH care delivery model.
• Collaborate with the sales team to identify educational needs and improve patient transition volume.
• Maintain patient confidentiality and comply with legal guidelines.
• Document education using OCH applications and technology.
• Coordinate transitions from hospital to home or alternate care settings in collaboration with facilities and providers.
