The position requires an Active RN license, 2+ years in healthcare, and the ability to travel locally to partnerships facilities and hospitals for in-person education and support.

Qualifications

• Active and unrestricted RN license required.
• Minimum of 2 years in healthcare industry.
• Strong communication and relationship-building abilities.

Responsibilities

• Educate patients, caregivers, and facility staff about OCH services and products for successful transition to in-home or alternate care.
• Perform patient assessments to determine eligibility for the OCH care delivery model.
• Coordinate with sales to identify educational needs and improve patient transition volume.
• Maintain confidentiality and comply with legal guidelines for patient information.
• Document education using OCH applications and technology.
