Job summary

Option Care Health, Inc. is seeking a Clinical Transition Educator (RN) to educate patients and care teams about home-based infusion services. This role partners with the sales team to drive patient transitions to Option Care Health care delivery.

Requires an active RN license and a minimum of two years in healthcare. The position involves 100% local travel to facilities and homes to support discharge planning and education, with documentation in OCH systems.

Qualifications

Active and unrestricted RN license.
Minimum of 2 years of healthcare experience.
Experience establishing relationships with individuals at various organizational levels.
Proficient with MS Office (Word, Excel, PowerPoint).
Willingness to travel locally to care facilities, hospitals, and patient sites.

Responsibilities

Educate patients, caregivers, and facility staff about OCH services and products for home or alternate site delivery.
Conduct patient assessments to determine suitability for the OCH care model.
Collaborate with the sales team to tailor clinical transition education and increase patient transitions.
Protect confidentiality and comply with privacy laws for patient information.
Document education activities using OCH applications and systems.
Coordinate hospital-to-home transitions with facilities, providers, and internal teams.
Provide in-person education and gather pre-admission and insurance information to initiate services.
