Job summary

Option Care Health is seeking a Clinical Transition Educator to deliver clinical education and training for successful patient transitions to our care model. You will partner with the sales team to increase patient transitions to OCH care delivery.

The role requires an active RN license, at least 2 years in healthcare, and ability to travel locally. You will educate patients and caregivers, coordinate with facilities, and document education using OCH systems.

Qualifications

• Active and unrestricted RN license.
• Minimum of 2 years of experience in the healthcare industry.
• Ability to travel locally 100% as required by role.
• Strong communication and presentation skills.
• Familiarity with ACHC, URAC standards.

Responsibilities

• Educate patients, caregivers, and facility staff about OCH services at home or alternate sites to ensure successful transition.
• Assess patient viability for transition to the OCH care delivery model.
• Coordinate with the OCH sales team to identify clinical transition education needs and improve patient transition volume.
• Maintain confidentiality of patient and proprietary information and comply with privacy laws.
• Document education using OCH applications and technology.
• Coordinate transitions from hospital to home or other care settings with facilities, providers, and internal teams.
• Provide in-person education and gather insurance information to initiate services.
