Job summary

Option Care Health Inc. is seeking a Clinical Transition Educator to train patients, caregivers and facility staff on home infusion services and the care delivery model.

The role partners with the sales team to grow patient transitions and coordinates with hospital staff to ensure safe discharge and continuity of care. Active RN license is required, with at least two years in healthcare and strong relationship-building and communication skills.

Qualifications

• Active and unrestricted RN license required.
• Experience delivering education and training to patients, caregivers, and staff.
• Ability to travel locally to care facilities and hospitals.

Responsibilities

• Educate patients, caregivers and facility staff about OCH services and home infusion delivery.
• Assess patient eligibility and coordinate with sales to drive transitions.
• Document education using OCH applications and ensure continuity of care.
• Collaborate with hospital staff and internal teams to support timely discharge and education.
