Job summary

Option Care Health is seeking a Clinical Transition Educator to deliver clinical education and training for seamless patient transitions to our home infusion care model. You will partner with the sales team to increase the number of patients transitioning to OCH care delivery.

Responsibilities include patient assessments, coordination with facilities and internal teams, documentation of education, and ensuring timely discharge and continuity of care.

Qualifications

• Active and unrestricted RN license required.
• Minimum of 2 years healthcare experience.

Responsibilities

• Evaluate, educate, and train patients, caregivers, and facility staff about OCH services in home, in‑home, or virtually to ensure successful transition to OCH care delivery.
• Conduct patient assessments to determine eligibility for the OCH model.
• Coordinate with the OCH sales team to identify educational needs and improve patient transition volume.
• Maintain confidentiality of patient and proprietary information per legal guidelines.
• Document education using OCH applications and technology.
• Coordinate transitions from hospital to home or alternate care settings with staff and providers to ensure timely discharge and continuity of care.
• Provide in‑person education, gather insurance information, and facilitate initiation of services for safe, effective home care.
