Job summary

Jobtailor in Massachusetts is seeking an experienced registered nurse to provide personalized, in-home care, manage chronic conditions, and help patients remain independent.

You will manage a caseload, organize daily workflow with clinical team support, assess patients, develop care plans, and coordinate with physicians. Travel throughout the territory is required, with emphasis on documentation and care coordination.

Qualifications

• Current, unrestricted RN license.
• BSN preferred as an optional credential.
• At least one year of clinical nursing experience.
• Home health, hospice and Homecare Homebase experience preferred.

Requirements

• Strong assessment, communication, and care-coordination skills


• Valid driver’s license, dependable transportation, and auto insurance


• Ability to travel throughout the assigned territory


• Ability to sit, stand, bend, lift and move intermittently and lift 50–100 lbs.
