Job summary

Elara Caring is seeking an experienced Registered Nurse to manage and coordinate home health care for a diverse caseload. You will conduct assessments, develop and implement care plans, and collaborate with a multidisciplinary team to optimize patient outcomes.

You will travel within the branch area (up to 50%) to perform visits, evaluations, and follow-ups, ensuring HIPAA compliance and high-quality patient care in home settings.

Qualifications

• Current, unrestricted state RN licensee.
• 1+ year experience in a clinical care setting as a nurse.
• Able and willing to travel within branch/office coverage area.
• Ability to frequently lift, push, pull, and support up to 50 pounds.
• Must possess a valid driver's license or reliable transportation.
• Must be able and willing to travel 50%.
• Associates Degree or Bachelor’s Degree in Nursing is preferred.
• Previous Home Health or Hospice Experience is preferred.
• Previous experience with HomeCare HomeBase is preferred.

• Assume primary responsibility for caseloads, including planning and evaluating plans of treatment.
• Assess patient needs and incorporate physical, psychological, social, and spiritual factors.
• Conduct initial evaluations and re-evaluate patients’ nursing needs during visits.
• Make follow-up visits according to patient, family, and facility needs.
• Communicate significant findings to supervisor, physician, facility, and team.
• Revise plans of care in response to issues and notify team.

Job description

• Ensure all activities align with the vision of Elara Caring’s board of directors, executive team, and Home Health leadership
• Maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule
• Assess assigned caseloads of home health care patients and families to identify physical, psychosocial, and environmental needs
• Assume primary responsibility for caseloads, including assessing, planning, coordinating, implementing, and evaluating plans of treatment
• Assess patient needs and incorporate physical, psychological, social, and spiritual factors into plans of care
• Conduct initial evaluations and re-evaluate patients’ nursing needs during each visit
• Make follow-up visits according to patient, family, and facility needs
• Communicate significant findings, problems, and changes in health condition or environment to the Clinical Supervisor, physician, facility, and other personnel
• Revise plans of care in response to patient care issues and notify the Clinical Supervisor, physician, facility, and other team members

Requirements

• Current, unrestricted state RN licensee
• 1+ year experience in a clinical care setting as a nurse
• Able and willing to travel within branch/office coverage area
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
• Must be able and willing to travel 50%
• Associates Degree or Bachelor’s Degree in Nursing is preferred
• Previous Home Health or Hospice Experience is preferred
• Previous experience with HomeCare HomeBase is preferred
