Job summary

Jobtailor is seeking an experienced registered nurse to provide home health care in patients’ homes in the Chicago area. You will assess caseloads, plan and coordinate comprehensive care, and reevaluate nursing needs during visits.

Ideal candidates have 1+ year in clinical settings, an active RN license, and willingness to travel up to 50%. Proficiency with HomeCare HomeBase is preferred and HIPAA privacy rules apply throughout patient care.

Job description

• Provide home health nursing care to patients in their preferred home setting
• Assess assigned caseloads of home health care patients and families
• Identify physical, psychosocial, environmental, psychological, social, and spiritual patient needs
• Assume primary responsibility for caseloads, including assessing, planning, coordinating, implementing, and evaluating plans of treatment
• Perform initial evaluations and reevaluate patients' nursing needs during each visit
• Conduct follow-up visits according to patient, family, and facility needs
• Communicate significant findings, problems, and health-condition changes to the Clinical Supervisor, physician, facility, and other care personnel
• Revise plans of care in response to patient-care issues and notify relevant clinical team members
• Maintain patient and staff privacy and confidentiality pursuant to HIPAA requirements
• Report to the Clinical Manager or Clinical Supervisor

Requirements

• Current, unrestricted state RN licensee
• 1+ year experience in a clinical care setting as a nurse
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Able and willing to travel within branch/office coverage area
• Dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws
• Must be able and willing to travel 50%
• Associates Degree or Bachelor's Degree in Nursing is preferred
• Previous Home Health or Hospice Experience is preferred
• Previous experience with HomeCare HomeBase is preferred
• Maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule
